NCPH Book Award Submission

Provide the name of the book you are nominating for this award and full contact information - including name, address, and email - for the nominator/main contact and all award nomination participants (each author). We require a CV or resume for each author, as well. At the end of this form we also request that you indicate how and when the physical book is being shipped.

Please note: only the nominator/main contact will be notified if the submission does not receive the award. Self-nominations are welcome.
