NCPH Excellence in Consulting Award Submission

Provide the name of the project and individual or group you are nominating for this award and full contact information - including name, address, and email - for the nominator/main contact and all award nomination participants (each person who is involved in the project or group). We require a CV or resume for each individual, as well.

Please note: only the nominator/main contact will be notified if the submission does not receive the award. Self-nominations are welcome.

Your completed form will be sent to the NCPH office and each of the Excellence in Consulting Award Committee members listed here. If you have any physical material that must be sent (ex. book, DVD, etc.), please send it to each of the committee members and the NCPH office at:
