NCPH Student Project Award Submission

Provide the name of the project and each individual you are nominating for this award and full contact information - including name, address, and email - for the nominator/main contact. We require a CV or resume for each nominee, as well.

Please note: only the nominator/main contact will be notified if the submission does not receive the award. Self-nominations are welcome.

Your completed form will be sent to the NCPH office and each of the Student Project Award Committee members. If you have any physical material that must be sent (ex. book, DVD, etc.), please send it to each of the committee members and the NCPH office at:
